This form is for students who have been invited to participate in our school-based mentoring program. A mentor is a caring adult who will meet with you regularly at school to listen, encourage, and support you with your goals. Your answers to the following questions will help us get to know you better so that we can match you with a mentor who will be a good fit for you.
