Referring Students for Mentoring Support
SSV is excited to partner with your school to match your students with caring adult mentors. We use a simple referral and enrollment process designed to ensure that students and families are informed, interested, and set up for a positive experience.
Step 1: Student Referral
Identify students who may benefit from mentoring and submit a Student Referral Form.
Please refer to Guidelines for Choosing Students for Mentoring.
Step 2: Family & Student Engagement
Students who are interested and engaged are more likely to benefit from a mentoring relationship. Once a student is referred, families and students should be invited to learn more about the program before completing the required forms.
- Mentoring Program Overview for Families: Share this short guide to help families understand what their child is being referred for and what we need from them.
- Student Interest Form: Students are asked to complete a short form to confirm their interest and share what they hope to gain from the experience.
- Parent/Guardian Permission: In accordance with the Family Educational Rights and Privacy Act (FERPA), we require parent consent before any student can be matched with a mentor. Please ensure that a consent form (available in English and Spanish) is completed and submitted for each student being referred.
